MEMORY HEALTH PROGRAM COORDINATOR PART-TIME 16 HOURS TUESDAY/THURSDAY POSITION SUMMARYThe Memory Health Program Coordinator supports and coordinates memory care services for Residents with cognitive changes, promoting their physical, social, and emotional well-being. This role assists in delivering memory-related programs, provides guidance to Team Members, and collaborates with the Manager of Memory Health Services to ensure best practices in dementia care across the continuum. Job Specific DutiesReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Delivers person-centered, evidence-based programming that supports cognitive and emotional well-beingAdvocates for Residents and communicates effectively with families, Team Members, and other departmentsAdapts programming to Resident needs and provides Team Members with tools and strategies for effective memory careMonitors and reports on changes in Resident behavior or condition to appropriate Team MembersCollaborates with the Memory Health Manager and other departments to ensure coordinated careBuilds trusting relationships with Residents and families to support quality of lifeAssists in implementing, maintaining, and evaluating memory care programsPrepares reports and documentation to inform decision-making and support team communicationIdentifies program needs and contributes to ongoing improvementsCreates a welcoming, personalized experience for each ResidentPromotes engagement by tailoring activities and interactions to individual interests and abilitiesMeets department uniform, appearance, and grooming requirementsComplies with all OSHA safety regulations, applicable local laws regarding health, safety or environment Required Skills and Abilities Effective communicator in one-on-one and small group settingsStrong organizational, time management, and record-keeping skillsDemonstrates sound judgment and decision-making in routine situationsWork professionally with people from a wide variety of cultural and socio-economic backgroundsMust possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general publicWhen working in Resident areas, must be able to communicate effectively in EnglishComplies with protection of health information (HIPAA) safeguardsAttends and participates in in-service educational classes, Healthcare Academy and staff meetings as directed Educational and ExperienceHigh school diploma or equivalent requiredExperience in senior living and/or dementia care preferredWillingness to complete dementia-related trainingUses basic Microsoft programs (Word, Excel), email, and office equipmentBasic knowledge of dementia and cognitive decline Physical RequirementsThe physical demands described here are representative of those that must be met by a Team member to successfully perform the essential functions of this job.
